Request a correction or record review
Use this form to report inaccurate information, submit an update, ask for a hide review, or request sealed or restricted-status evaluation. Original supporting documents help the moderation team review publication status faster.
Before you submit
Provide the record URL or enough identifying information to locate the record.
Explain what changed and why the current listing should be corrected, updated, hidden, restricted, or sealed.
Include supporting documentation when available. Submission does not guarantee removal, but it does create a review ticket in the moderation queue.
